Sister Margaret Ann Pahl, a 71-year-old nun who served as caretaker of the chapel at Mercy Hospital in Toledo, Ohio, was found dead on Holy Saturday, April 5, 1980. She had been strangled and stabbed 31 times, including nine wounds arranged in the shape of an inverted cross. Prosecutors later argued this pattern was deliberate and intended to humiliate her in death. Pahl's body was found covered with an altar cloth, with her clothing and body arranged in a manner suggesting sexual assault, although it was not established that a sexual assault had occurred. Some elements of the crime scene led investigators to consider the possibility of ritualistic motives.

Father Gerald Robinson, the hospital chaplain who ministered to patients there, was questioned about the killing in 1980 but was not charged at the time. He presided over Pahl's funeral Mass four days after her death. According to a former chief of the criminal division in the Lucas County prosecutor's office, Dean Mandros, a deputy police chief broke off an interview with Robinson roughly two weeks after the killing and allowed a monsignor to escort him from police headquarters, a departure from normal procedure that reportedly upset detectives. Mandros also said the deputy chief asked detectives for their case reports, some of which were never seen again, leading to allegations of a cover-up.

The case remained unsolved for more than two decades. In 2003, police received a letter from a woman using the pseudonym "Survivor Doe," who alleged that Robinson had sexually abused her as a child as part of a pattern of ritual abuse. She later filed a civil lawsuit against Robinson, which was dismissed in 2011 as untimely. Her allegations prompted a review by the prosecutor's cold case unit. Forensic testing on a sword-shaped letter opener recovered from Robinson's apartment indicated it was consistent with the weapon used in the killing; a prosecution expert said the weapon could "not be ruled out," and the tool's tip reportedly matched a wound in the victim's exhumed jaw. Investigators also located three witnesses who reported seeing Robinson near the chapel around the time of the killing.

Robinson's trial began April 24, 2006, and he was convicted on all counts on May 11, 2006. This was reported as only the second homicide conviction of a Catholic priest in the United States, after Hans Schmidt in 1916. Robinson's conviction was upheld by Ohio's Sixth District Court of Appeals in 2008, and the Ohio Supreme Court declined to hear his appeal that December. A further post-conviction petition was denied and that denial was affirmed by the appellate court in February 2013. Robinson suffered a heart attack in May 2014 while further appeals were pending and was transferred to the hospice unit of a state prison medical facility in Columbus, Ohio, where he died on July 4, 2014.
