Babita Deokaran worked as the acting chief director financial accounting officer within the Gauteng Provincial Government Department of Health. She had been on special leave for more than a year before her death while a Special Investigating Unit (SIU) probe examined an alleged COVID-19 PPE procurement scandal valued at approximately R332 million (about US$20 million) within the department. An SIU official confirmed that Deokaran was a key witness in efforts to expose a syndicate benefiting from this state corruption.

On 23 August 2021, at around 08:00 SAST, Deokaran was shot and killed after dropping her daughter off at school and returning home. A vehicle pulled alongside hers and multiple shots were fired after she had parked outside her house in Johannesburg South, Gauteng Province. Her death drew significant public attention in South Africa due to her status as an anti-corruption whistleblower and the absence of state protection afforded to her.

A neighbour reported having seen a suspicious BMW at the location on multiple prior occasions, which helped police trace the vehicle to a former member of the South African National Defence Force based near Pretoria, leading to the arrest of six suspects. A passenger who was in Deokaran's vehicle at the time also provided evidence used in the investigation. Media reporting indicated Deokaran had been stalked for over a month before the killing, that CCTV cameras nearby had been disabled shortly before the attack, and that sophisticated surveillance software may have been used to track her phone and movements. The six suspects — Nhlanganiso Ndlovu, Siphakanyisa Dladla, Zitha Radebe, Simphiwe Mazibuko, Sanele Mbele, and Phakamani Radebe — were arrested within days of the attack and were reported to have been paid hired assassins, with one report putting the payment at around R2 million. Following her death, the chief executive of Tembisa hospital and the provincial Department of Health chief financial officer were suspended.

In the Johannesburg High Court, the six men were sentenced to a combined 95 years' imprisonment on the eve of the second anniversary of the killing. Sentences included 22 years for two defendants, 15 years for two others, a combined 15 years for one defendant on murder and firearm charges, and 6 years for the remaining defendant. The convicted men implicated three further individuals; one of them was killed in Germiston about four months after Deokaran's murder.

As of the SIU's most recent public statements, the investigation into the murder and related procurement syndicates remains ongoing, with officials describing efforts to finalise evidence and identify those who organised the killing. Reporting has also examined broader allegations of tender-related looting at Tembisa Hospital and related entities, and raised questions about earlier police handling of a suspect. President Cyril Ramaphosa stated after her death that greater protection should be extended to whistleblowers.
