On the evening of 7 April 2022, at around 9:00 p.m., a shooter dressed in black clothing opened fire at three locations on Dizengoff Street, a major thoroughfare in central Tel Aviv, including near the Ilka Bar. Ten people were taken to Ichilov Hospital. Two men, both aged 27, died of their injuries that night. A day later, 35-year-old former Olympic kayaker Barak Lufan, who had been injured in the attack, died of his wounds, bringing the total death toll to three. Six other people were wounded, including three critically.

The gunman fled on foot and evaded capture for approximately nine hours. Around 1,000 soldiers and police officers took part in the manhunt, including special forces from Sayeret Matkal, LOTAR Eilat, and Shaldag. Tel Aviv residents were told to stay indoors and away from windows and balconies. In the early morning of 8 April, the shooter was located hiding in a mosque in Jaffa and was killed in a gunfight with Shin Bet agents and Israeli police Yamam officers.

The perpetrator was identified as Raad Fathi Khazem, a 28-year-old Palestinian from Jenin in the northern West Bank. He had prayed at a mosque on Yefet Street before the attack and was later killed at that same mosque. His ties to any militant group were unclear. According to information released by the Israel Defense Forces on 10 April, Khazem had been involved in a financial dispute with neighbors in Jenin who had threatened his life and had reportedly tried to shoot him, which led him to acquire a firearm. His father, Fathi Khazem, was a retired security officer for the Palestinian Authority who had previously served time in Israeli prison and who publicly praised the attack carried out by his son. Khazem's brother, Abed, was killed on 28 September 2022 when their father's home was struck by a missile during an Israeli raid on Jenin, in which three other people also died.

The three people killed were Tomer Morad and Eytam Magini, both 27 and from Kfar Saba, and Barak Lufan of Giv'at Shmuel, a former Olympic kayaker who had represented Israel at the 2008 Beijing and 2012 London Olympics.

In the aftermath, thousands of Palestinians in Jenin gathered near the shooter's family home in a show of support, and Palestinian Authority President Mahmoud Abbas condemned the killing of Israeli civilians, while a Fatah-affiliated local official praised Khazem. Hamas and Islamic Jihad did not claim responsibility but expressed support for the attack. U.S. and EU officials condemned the shooting. The Israel Defense Forces subsequently entered Jenin, raided Khazem's family home to prepare it for demolition, and Israeli Prime Minister Naftali Bennett authorized expanded security operations in the West Bank. On 9 April, the IDF killed a Palestinian man affiliated with Islamic Jihad during a related operation, which local authorities said also wounded 13 others.
